Do you need an intake? Intake forms help us create positive programs and safe environments for all participants.
If your child is enrolling in a Social Group and has been enrolled in an Accord program within the past two years, you do not need an intake.
If you are registering for activities, clubs, fitness, workshops, and events, you do not need an intake. However, if your child exhibits aggressive behaviors, please complete an intake form.
If you are new to Accord and are registering for a Social Group, please complete an intake form.

An Accord Center staff member will contact you to schedule a short follow-up phone call.
If the Accord Center programs are a good fit, you will be able to register for programs. Otherwise, we will do our best to provide you with appropriate resources to help you and your child.
